WWE Raw 25 Years special edition aired on January 22, 2018, and McCool was recognized as part of a section that included other women who had made significant contributions to the company’s success. The next week, as a face, she was a surprise entry in the first women’s Royal Rumble match at the 2018 Royal Rumble, where she made the most eliminations in the event, five, until being eliminated by Natalya at the conclusion of the match. McCool competed in a battle royal at the WWE Evolution pay-per-view, which was held in October and featured only female wrestlers. Ember Moon was the one that knocked her out of the contest.

McCool replaced Layla in her Women’s Championship match against Tiffany on the July 30 edition of SmackDown!, and she defeated her and retained the championship. McCool beat Melina at Night of Champions to unify the WWE Women’s Championship with the WWE Divas Championship, becoming the first WWE Unified Divas Champion.

McCool and Layla both had their respective championship belts when the match ended. Her title was stripped from her at Survivor Series on November 21 when Natalia beat McCool and Layla in a two-on-one handicap match using a handicapping system. During TLC: Tables, Ladders, and Chairs, Natalya and Beth Phoenix defeated McCool and Layla in the inaugural Divas tag team tables match, which was broadcast live on the WWE Network.

In January 2011, LayCool exercised its rematch clause, which resulted in the scheduling of a two-on-one handicap match at the Royal Rumble in New Orleans. During the event, however, the match was changed to a fatal four-way due to the inclusion of Eve Torres, who ultimately took first place.

In the following year, McCool teamed up with Layla to create LayCool, and the pair engaged in a contentious rivalry with Mickie James, during which LayCool tormented James and made fun of her weight. After a five-on-five Survivor Series battle at the 2009 Survivor Series, which James’ side won after Melina pinned McCool, the rivalry culminated in a WWE Championship bout in 2010. Later, at TLC: Tables, Ladders, and Chairs, James emerged as the number one challenger for McCool’s reign as the WWE Women’s Champion, which McCool ultimately maintained with the help of Layla following interference from the former champion.

At the Royal Rumble, McCool lost the Women’s Championship to Nia Jax; however, she was able to reclaim it on February 26, 2010, edition of SmackDown! when special guest referee Vickie Guerrero interfered, making her the first woman to hold the title twice. McCool and Layla beat Beth Phoenix and Mickie James on the April 23 edition of SmackDown!, and after the match, they assaulted Phoenix.

As a result of this, McCool was defeated by Phoenix in an “Extreme Makeover” bout at Extreme Rules, resulting in him losing the title. When Phoenix challenged McCool and Layla in a Women’s Belt two-on-one handicap match on May 14th, Layla was able to pin Phoenix and win the championship for the first time in the history of SmackDown! Despite Layla’s victory, McCool and Layla were credited for cutting the Women’s Championship in half, and the pair were unofficially referred to as the “co-champions.”

The couple had been together since McCool’s high school days when she met Jeremy Louis Alexander. In 2006, the couple decided to separate. The wedding took place on June 26, 2010, in Houston, Texas, and was attended by her fellow wrestler Mark Calaway, better known by his ring as The Undertaker. Their first kid together was born in August 2012, and she is a girl.

McCool went on to beat Gail Kim on the May 22, 2009, episode of SmackDown to earn the title of WWE Women’s Champion as the number one challenger for the title. Alicia Fox assisted McCool in defeating Melina at The Bash, allowing her to win her first Women’s Championship a month later. McCool became the first Diva in history to win both the WWE Divas and the WWE Women’s titles with her victory. When she faced Melina again on July 26 at Night of Champions, she was victorious, and she also retained the belt against her in a Lumberjill match on the October 2 edition of SmackDown!

McCool defeated Natalya in a Golden Dreams match on the July 4, 2008, edition of SmackDown!, advancing her to The Great American Bash to face Natalya for the WWE Divas Championship. McCool beat Natalya in the event, which took place on July 20, to become the first-ever WWE Divas Champion. On SmackDown in September and at the Unforgiven pay-per-view in November, McCool successfully defended the title against Maryse, and on SmackDown in December, she successfully defended the championship against Maria.

McCool was defeated by Maryse in the Divas title match on the December 26 edition of SmackDown! Following the bout, McCool turned heel and attacked Maria, the special guest referee who had presided over the contest, accusing her of being to blame for the defeat. In the following weeks, McCool also assaulted Eve Torres, which resulted in Torres’ professional wrestling debut, which McCool took care of.

Soon after, she started working as a valet for the pair of K. C. James and Idol Stevens, who was known as “The Teacher’s Pets” as a result of her affiliation with them. The three started a battle with Paul London and Brian Kendrick, as well as their valet Massaro, for the WWE Tag Team Championship, however, they were unsuccessful in their pursuit of the championship. The rivalry came to a conclusion when London and Kendrick defeated them in a match at No Mercy back in October.

McCool was admitted to the hospital on November 28 after suffering from an enlarged kidney, a cracked sternum, and an electrolyte imbalance. She was discharged from the hospital on December 2nd, after a brief stay. SmackDown! featured McCool as a member of a 10-Diva tag team match on March 30, 2007, and she made a triumphant comeback on April 1, 2007. McCool re-established herself as a fan favorite on the April 13 edition of SmackDown! when she rushed to the assistance of Ashley after she was assaulted by Jillian Hall in the backstage area. As a result, a series of battles took place between Victoria and McCool, including one in which Victoria and Kenny Dykstra beat McCool and Chuck Palumbo in a mixed tag team match.

When she was a WWE wrestler and Divas, she participated in six episodes of the game show Family Feud during the week of November 5, 2007. As well as appearing on Project Runway with Maria Kanellis and Candice Michelle on February 6, 2008, she also participated in an episode of The Bachelorette with Kristal Marshall, Layla El, and Torrie Wilson on February 6, 2008. She had an appearance alongside John Cena on The Best Damn Sports Show Period, which aired on June 3, 2008. McCool also had an appearance in the January 2009 issue of Muscle & Fitness, where she was joined by Eve Torres and Maryse. A special WWE version of the game show Is You Smarter Than a Fifth Grader included her as well.

She has had multiple wrestling-related injuries, the most recent being a broken nose in November 2007 while on a WWE foreign tour when Victoria’s clothesline struck her in the face during a match. In addition to having two broken ribs, a fractured sternum, and two broken xiphoid processes, she has also been hospitalized twice.

When she ultimately chose to retire from active wrestling competition and, as a result, departed the WWE and terminated her active wrestling career, she revealed that her foot had been damaged for two months earlier, with a fractured toe, ruptured joint capsules, and a torn medial collateral ligament (MCL). She was diagnosed with skin cancer in 2016, and she had effective treatment at the time. She made a public admission in April 2019 that she had battled with infertility and had experienced many miscarriages throughout her pregnancy.
