Cox received the E. K. Janaki Ammal Award from India and is a foreign member of the Stockholm-based Swedish Royal Academy of Agriculture and Forestry. Cox was designated one of the 11 “Heroes of Medicine” by TIME magazine in 1997 for his efforts in ethnobotanical medication discovery. In the same year, he was given the Goldman Environmental Prize for his work to save Samoan rain forests, and he and Senator Tom Harkin later shared the Rachel Carson Award. He was named President of both the Society of Economic Botany and the International Society of Ethnopharmacology. He served as Director of the National Tropical Botanical Gardens in Hawaii and Florida for seven years.

Cox created and serves as chairman of Seacology, a Berkeley, California-based environmental nonprofit that has saved 98,000 acres of rainforest and 1.7 million acres of coral reef on islands throughout the world.
